Home Medicines Review

A Home Medicines Review (HMR) is a free service aimed at optimising medication use and reducing adverse medication events by a credentialled pharmacist visiting a patient’s home for a discussion about their medicines. Qualified practitioners like GPs, Pain Medicine Specialists, and others can refer patients for an HMR, which involves collaboration between the patient, referring medical practitioner, accredited pharmacist, and wider healthcare team.

During the HMR interview, the accredited pharmacist works with the patient in their home to gain an understanding of how their medicines are used and this can also provide an opportunity for medicines education. Following the interview, a comprehensive HMR report is generated, providing insights and recommendations to develop a personalised medication management plan. This report is shared with the referring practitioner, GP (if different), and other healthcare team members, as requested by the patient, and may be uploaded to the patient’s My Health Record. Through this process, pharmacists can assist in simplifying and optimising your medicines.
